How to View Form Results in Simulation Reports
After participants complete forms during a simulation, you can view their responses in the simulation report. This guide will show you how to access and understand form results.
Understanding Form Results
Form results show you:
- All questions from the form
- Participant answers to each question
- Completion timestamp (when the form was submitted)
- Form title
Forms can appear in two places on the simulation's Activity steps timeline:
- Before the simulation: Forms completed before the simulation chat begins
- After the simulation: Forms completed after the chat ends, as part of the report flow
Viewing Form Results in Individual Simulation Reports
Step 1: Access the Simulation Report
- As an instructor:
- Open the space and click Results on the simulation's card
- Go to the Participants tab and find the participant
- Click View Report on the run you want to review
- As a participant:
- Open History in the sidebar and open the run
- Good to know: A report can be opened once the run is completed (or abandoned). If you have a direct link to a simulation report, you can open it directly
Step 2: Find the Forms Tab
- Look for the Forms Tab:
- In the simulation report, you'll see several tabs at the top
- Common tabs include: Feedback, Feedback Chat, Simulation Chat, Quizzes, and Forms
- The Forms tab only appears if the participant completed at least one form
- Click the Forms Tab: Click on the "Forms" tab to view all form results. If the results can't be loaded, the tab says so and offers to try again
Step 3: Review Form Results
When you open the Forms tab, you'll see the heading Form Results and:
- Form Cards: Each form completed by the participant appears as a separate card
- Form Title: The name of the form (e.g., "Pre-Simulation Survey")
- Completion Date: When the participant submitted the form, shown as Completed: followed by the date and time
- Questions and Answers: All questions with their corresponding answers
Question Display:
- Questions are shown with their full text
- Answers appear below each question
- When a participant picked several options, they are displayed as a comma-separated list
- Long text answers are shown in full
Step 4: Understanding the Results
Answer Formats:
- Text Questions: Answers appear as typed text
- Choice Questions: The selected options are shown by their option text
- Rating Scales: The selected rating is displayed
Empty Answers: If a participant didn't answer a question, you'll see No answer.
Viewing Form Analytics (Aggregated Results)
For a broader view of form responses across many runs, use the Form Analytics tab.
Step 1: Open Form Analytics
- For one space: Open the space, click Results on the simulation's card, and open the Form Analytics tab
- Across every space that uses the simulation: In the Library, open the simulation card's ⋮ menu, choose View Report, and open the Form Analytics tab. This page also has a time period filter
Step 2: View the Overview
- Summary figures: Total Submissions, Unique Users, and Completion Rate
- The Form List shows the same figures for each form in the Simulation
- Click View Details on a form to analyze it
Step 3: Analyze Specific Forms
- Answer Analysis: A table with each question's Total Answers, Most Common Answer and its Count, plus the Average and Std Dev for numeric answers
- Answer Distributions: Use Select Question to chart how participants answered one question, with bar and pie charts for choice questions, a rating distribution for ratings, and a table for matrix questions
- Click Back to Form List to pick another form
Exporting Form Results
Form results can be exported along with other simulation data:
- Export Options: On the space's Results page, click Export Data and choose a format (Simple Table, Coded Forms, or Full Report). Under Include, keep Form answers selected. The file is an Excel workbook
- What's Included: The form questions and answers, next to each run's participant and other run data
See How Form Results Appear in Excel Exports for the layout.
Tips for Reviewing Form Results
Individual Reports
- Review in Context: Look at form results alongside the simulation chat and evaluation to get a complete picture
- Check Completion Times: Note when forms were completed relative to the simulation timeline
- Compare Responses: If multiple forms were completed, compare responses to see participant progression
Analytics View
- Time Period Filters: On the Library's View Report page, use the time period filter to analyze trends over specific periods
- Question-Level Analysis: Drill down into specific questions to understand response patterns
- Form Comparison: Compare analytics across different forms to identify which forms provide the most valuable insights
Troubleshooting
Forms Tab Not Appearing
If you don't see a Forms tab in a simulation report:
- Check if Forms Were Completed: The Forms tab only appears if at least one form was completed. Verify that the Simulation includes forms and that participants completed them
- Check Form Configuration: Ensure the form step is on the Simulation's Activity steps timeline and has a form selected
- Check Permissions: Ensure you have permission to view simulation reports. Contact your administrator if you believe you should have access
Missing Form Results
If form results are missing:
- Check Form Completion: Verify that participants actually completed the forms. Forms must be submitted to appear in results
- Check Form Status: Ensure the form still exists and is linked to the Simulation
- Refresh the Report: Try refreshing the report page, and clear your browser cache if needed
Incomplete Form Data
If you see incomplete form data:
- Check Required Fields: Some questions may not have been answered. This is normal if questions weren't marked as required
- Hidden Questions: If a question was hidden by a condition, its answer is not recorded
- Check Form Version: If a form was edited after responses were collected, older responses may not match the current form structure. Historical data is preserved even when forms are updated
Best Practices
Regular Review
- Review Form Results Regularly: Check form responses to understand participant needs and feedback
- Look for Patterns: Identify common themes or issues across multiple responses
- Act on Feedback: Use form responses to improve your Simulations
Data Analysis
- Use Analytics: Leverage the Form Analytics tab to understand trends across all participants
- Compare Time Periods: Use the time period filter on the View Report page to see how responses change over time
- Focus on Key Questions: Pay special attention to questions that provide actionable insights
Privacy and Security
- Respect Privacy: Form responses may contain sensitive information
- Secure Access: Ensure only authorized users can view form results
- Data Retention: Be aware of data retention policies for form responses
Next Steps
After reviewing form results:
- Analyze Patterns: Look for common themes or issues in responses
- Update Simulations: Use feedback to improve your Simulations
- Refine Forms: Adjust form questions based on what you learn from responses
- Share Insights: Communicate key findings with your team or stakeholders
