Ordering forms & questionnaires (and what you can't upload)
This article answers two common questions about forms and questionnaires: where they appear (and how to change the order), and whether you can upload an existing questionnaire.
Where forms appear in a simulation
A simulation runs in stages: the steps before the simulation (the intro), the simulation chat, and then the steps after it — your feedback report, a feedback chat, and any custom forms or quizzes. You build all of them on one Activity steps timeline in the editor, split by a fixed Simulation marker.
- To collect something before the chat, add a form above the Simulation marker.
- To collect something after the chat, add a form or quiz below the Simulation marker.
Changing the order (e.g. a questionnaire before the feedback)
In the simulation editor’s Activity steps tab, the steps are a list you can drag and drop into any order — including across the Simulation marker, which moves a step from one phase to the other. To show a questionnaire immediately after the simulation but before the feedback report, drag your form step above the Feedback step. Learners then see: chat → your questionnaire → feedback report.
Answers to hidden questions are cleared
If a question is hidden by a condition after the participant answered it, that answer is no longer submitted — what is on screen and what gets recorded now match. Previously the answer lingered in the session and disappeared only from the submitted data, with nothing on screen showing the loss. If you built a form that relied on the old behaviour, check it. See How to Create a Form.
Can I upload a questionnaire from Word?
No — questionnaires can’t be imported from a Word or PDF file. Forms are built in Cesura’s in-app form builder, where you add questions (multiple choice, text, rating, and more). If you have an existing questionnaire, recreate its questions in the builder. For a large or complex one, contact us through Contact support in the sidebar and we’ll help.
Adding a consent form
Every simulation already shows a standard consent / terms screen before it begins. If you need your own informed-consent wording, add it as a form above the Simulation marker and mark it required, so participants must agree before continuing.
